Incident Reporting

 

Below you will find detailed information and instructions to allow the classification of patient safety incidents in critical care. It is a pilot project being run by Dr Tony Thomas (tony.thomas@srft.nhs.uk) on behalf to the standards committee of the intensive care society.

The section contains:

Enclosed are files that allow you to classify critical incidents, they make most sense if the documents are viewed in the order shown. If you have any questions or would like to take part please email tony.thomas@srft.nhs.uk.

1. Invitation letter.

2. A PowerPoint presentation without any commentary used for explaining the thinking behind the project.

3. An example of a report of information provided back to a unit that has taken part in the study.

4. Denominator data for time period that incidents are reported over.

Example of completed denominator data for a trust with more than one critical care unit.

5. List of definitions used in classification.

6. New zipped access database.
